Behavioral Health Group Practices
You built a group practice one clinician at a time. Getting every clinician paid correctly shouldn't cost you your weekends.
You Built This One Clinician at a Time
It usually starts the same way. You opened a private practice, filled your caseload, and started a waitlist. Turning people away felt wrong, so you brought on an associate. Then another. A few years later you have a group practice with a real payroll, a front desk, and clinicians who count on you to get their pay right.
Most owners are still seeing clients twenty or more hours a week. The business side grew in the gaps between sessions. Pay gets figured out on Sunday night. The books get looked at when someone asks for them. It works until it doesn’t, and in this field the first thing to break is usually clinician pay.
Who We Work With
Who We Work With
Therapy and counseling groups, psychology practices, and psychiatry practices. Practices with LCSWs, LPCs, LMFTs, psychologists, psychiatrists, and psychiatric nurse practitioners on staff. Our best fit is a group with three or more clinicians.
Where You Are
Where You Are
You still carry a caseload. You have a mix of licensed and pre-licensed clinicians, maybe some on W-2 and some on 1099. Insurance and private pay both come in, and pay is calculated in a spreadsheet you built yourself.
Clinician Pay Is the Hard Part
Every pay run, someone has to count each clinician’s sessions and match them to the right rate in that clinician’s agreement. An intake may pay differently than a regular session. A 53-minute session may pay differently than a 38-minute one. Some agreements pay on what was billed and some pay on what was collected. Then come the late cancellations, the no-shows, the denied claims, and the insurance company that takes money back three months later.
Owners who do this by hand make mistakes. Not because they’re careless, but because the math is tedious and it never stops. Clinicians notice when a check is short. A pay error that takes five minutes to fix can take months to repair in trust, and good clinicians have plenty of other places they could work.
Clinician Pay
Clinician Pay
We calculate session-based pay every pay run using the compensation agreements you already have. We follow your terms exactly as written. We don’t design pay plans or advise on how clinicians should be paid, because clinician compensation comes with compliance rules and those decisions belong to you and your attorney.
Payroll That Follows Through
Payroll That Follows Through
The calculated pay goes straight into payroll through Gusto. W-2 clinicians, 1099 contractors, and front desk staff all get paid on time, in any state. That includes telehealth clinicians who live somewhere other than where your office is.
Why We Understand This Work
Marcella Nichols, our President, holds a master’s degree in clinical psychology. She studied the work your clinicians do and understands how a therapy practice actually runs. Caseloads, intakes, supervision for associates working toward licensure, and how clinicians are paid and classified. None of it needs to be explained to her.
Before taking over the firm her mother founded in 1995, she spent seven years in recruiting and HR at a large Southern California software company, starting as a Recruiter and becoming Talent Acquisition Lead. That job taught her what makes people stay and what makes them leave. Getting paid correctly and on time is near the top of the list.
We Speak the Language
We Speak the Language
Session codes, superbills, sliding scale fees, cancellation policies, payer clawbacks. You won’t spend your first month teaching us what a 90837 is or why an intake is billed differently.
Same People Every Month
Same People Every Month
Your work is handled by a small team of enrolled agents who learn how your practice runs. Not a call center. Tax returns, quarterly estimates, and any IRS or state notices are handled by people who already know your books.
Find Good Clinicians and Keep Them
Staffing is the other problem that keeps group practice owners up at night. Good clinicians are hard to find, and a bad hire can cost months of lost sessions and a difficult conversation at the end. Marcella built our Hiring & HR Support service from her years leading a recruiting team, so you get real help finding people instead of another job post that goes nowhere.
Behind all of it sit clean books. We close them every month and send a one-page summary. If your practice bills insurance, we check that what your billing system says was collected actually landed in the bank. If you’re months behind, we bring things current first. Then you can get back to the work you trained for.
Hiring & HR Support
Hiring & HR Support
Job descriptions, pay benchmarks, offer letters, onboarding paperwork, and an employee handbook. If someone we help you hire leaves within 90 days, we find a replacement.
Monthly Bookkeeping
Monthly Bookkeeping
Books kept in QuickBooks Online and closed every month. Insurance collections matched to bank deposits so missing payments get caught early instead of discovered at tax time.
