Bookkeeping for Los Angeles Med Spas and Medical Practices

Bookkeeping, payroll, and tax for Los Angeles med spas, aesthetic clinics, and independent medical practices. Remote service from a firm doing this work since 1995.

Bookkeeping for LA Practices

Los Angeles is the second-largest med spa market in the country, with plastic surgeons, cosmetic dermatologists, and laser clinics spread from Century City to Sherman Oaks. Most of these practices collect cash up front, sell packages and memberships, and stock retail skincare. That combination makes the books harder than they look. We focus on med spa and aesthetic clinic bookkeeping and the specific problems that come with it.

Baker’s Bookkeeping & Tax Services has handled bookkeeping and tax work since 1995. We’re based in Lenexa, Kansas, and serve LA clients remotely in QuickBooks Online and Gusto. Our president, Marcella Nichols, spent seven years in recruiting and HR at a large Southern California software company, so she knows the region’s pace and labor market firsthand.

Monthly Bookkeeping

Books closed every month with a one-page summary you can read in a few minutes. If your practice bills insurance, we check that what your billing system reports as collected actually reached the bank. Books that are behind get brought current first.

Profit by Treatment

Botox, fillers, laser, and body contouring each carry a different product cost and commission. We show what each treatment actually earns. Prepaid packages and memberships are recorded as revenue when the service is performed, so your income isn’t inflated by money you haven’t earned yet.

Two Entities, Two Sets of Books

California’s corporate practice of medicine rules lead many LA med spas and cosmetic practices to operate as a physician-owned medical corporation alongside a separate management company. We provide multi-entity bookkeeping that keeps each one clean and records the fees between them the same way every month.

Tax Returns and Planning

California taxes high earners heavily, so practice structure, owner pay, retirement contributions, and the timing of equipment purchases all matter. Enrolled agents prepare your federal and California returns, handle quarterly estimates, and respond to IRS or Franchise Tax Board notices.

Los Angeles Practices We Work With

LA’s healthcare community goes well beyond aesthetics. The city has a large base of independent physicians, therapy and psychiatry groups, and integrative medicine clinics across the Westside, the Valley, Downtown, and the Eastside. Many are owned by clinicians who are excellent with patients and never had training on the business side.

We provide healthcare bookkeeping services built for practices like these. Our CIO, Marc Eastin, is a practicing neurosurgeon who also owns a business, so we understand what it’s like to see patients all day and then face the books at night. Your work is handled by enrolled agents who learn your practice, not a call center.

Med Spas and Aesthetic Clinics

Med spas, laser clinics, cosmetic dermatology, and hair restoration practices in Koreatown, Brentwood, Studio City, and beyond. Retail skincare is subject to California sales tax, so we file on time and keep product sales separate from treatment revenue.

Physician and Surgical Practices

Plastic surgeons, dermatologists, and specialty groups near Cedars-Sinai, UCLA, and the medical offices throughout the city. We calculate provider pay each month from your production reports under the agreements you already have, and keep separate books when the partners own the building.

Behavioral Health Groups

Therapy, psychology, and psychiatry groups across LA. Paying clinicians correctly under session-based agreements is where most owners run into trouble. We calculate clinician pay every pay run from your existing agreements. Marcella’s master’s degree in clinical psychology means she knows how these practices actually work.

Direct Care and Integrative Medicine

Concierge doctors, functional medicine, hormone, weight-loss, and IV clinics. Many LA physicians leave employment to open cash-pay practices. We set up the entity, books, and payroll before you open, then track membership revenue and file sales tax on the supplements you sell.

Common Questions About This Area

Do you have an office in Los Angeles?

No. Our office is in Lenexa, Kansas, and we work with LA practices remotely. Documents come in through a secure portal and meetings happen by video. Our president spent seven years working in Southern California, so the region isn't new to us.

What does bookkeeping cost for an LA practice?

Monthly bookkeeping runs $500 to $1,500 per month depending on transaction volume and complexity. If your books are behind, catch-up is billed at 75% of the monthly fee for each month we bring current. You get a flat quote before we start.

Do you file California sales tax on skincare and retail products?

Yes. If your med spa or clinic sells skincare, supplements, or other retail products, we file your sales tax returns with the California Department of Tax and Fee Administration on time. We track product sales separately from treatment revenue so the right amount gets reported.

My med spa has a medical corporation and a management company. Can you keep books for both?

Yes. We keep separate books for each entity and record the fees between them every month. We don't design the legal structure or give legal advice about it. That belongs with your healthcare attorney. Our job is to keep the books accurate once the structure is in place.

Can you run payroll for California employees?

Yes. Payroll runs through Gusto, which handles California withholding and state filings. We pay front desk staff, injectors, clinicians, and providers, including anyone who lives or works in another state.
