Bookkeeping and Tax for Denver Medical Practices

Bookkeeping, payroll, and tax for Denver direct primary care, integrative medicine, med spa, and behavioral health practices.

Bookkeeping and Tax for Denver Practices

Denver has become one of the more active places in the country for clinicians who want to practice on their own terms. Doctors leave large health systems to open direct primary care and concierge practices. Nurse practitioners and entrepreneurs open med spas and functional medicine clinics. Therapists who started solo build group practices with a handful of clinicians. Most of these owners are running a business for the first time, and the finances get complicated faster than they expect.

We’re a bookkeeping and tax practice based in Lenexa, Kansas, and our remote division works with independent practices across the country, including here in Denver. We handle monthly bookkeeping, payroll, and the year-round tax services for physicians and practice owners that go beyond filing a return. Tax work and IRS or state notices are handled by enrolled agents who learn your practice, not by a call center.

Monthly Bookkeeping

Books closed every month in QuickBooks Online, with a one-page summary that shows where the practice stands. Membership revenue, retail sales, and insurance payments are each recorded where they belong. If you bill insurance, we check that what your billing system says was collected actually reached the bank.

Opening Your Own Practice

Leaving employment to open a practice means choosing a business structure and setting up books, payroll, and software, all before the first patient walks in. Our new practice setup covers each of those pieces, so you open with systems that already work.

Colorado and Denver Payroll

Payroll for front-desk staff, clinicians, and providers runs through Gusto. That includes Colorado FAMLI premiums, state withholding, and Denver’s Occupational Privilege Tax for employees who work in the city. If you have providers or remote staff in other states, we handle that too.

Sales Tax on Retail Products

Denver collects its own sales tax separately from the state, and many surrounding cities do the same. If you sell supplements, skincare, or other retail products, we file in every jurisdiction where you owe so nothing slips through.

Denver Practices We Work With

The practices we work with in Denver are spread across the city and the metro. Membership-based primary care offices in Lowry and Central Park. Med spas in Cherry Creek and the Highlands. Therapy groups in Capitol Hill and Washington Park. Integrative clinics out toward Lakewood, Englewood, and Aurora.

What these owners have in common is that they trained to care for patients, not to run payroll or reconcile accounts. Our President, Marcella Nichols, holds a master’s in clinical psychology and spent years in recruiting and HR. Our CIO, Marc Eastin, is a practicing neurosurgeon who also owns a business. We know what it’s like to be the client, and we built the practice around that.

Direct Primary Care and Concierge

Denver has a strong direct primary care community, and it keeps growing. Membership revenue is steady, but it needs to be tracked correctly alongside dispensing and lab costs. We work with direct care and integrative medicine practices from the planning stage through their first few years and beyond.

Functional and Integrative Medicine

Hormone clinics, weight-loss programs, IV hydration lounges, and functional medicine practices. These businesses mix memberships, packages, and supplement sales. We separate service revenue from product revenue and handle the sales tax that comes with selling retail.

Med Spas and Aesthetic Clinics

Prepaid packages and memberships are money collected for treatments not yet delivered, so we track them instead of counting them as income right away. We show profit per treatment after product cost and commissions. If your med spa is split into a management company and a medical entity, we keep separate books for each.

Behavioral Health Groups

Therapy and counseling groups, psychology practices, and psychiatry practices. Paying clinicians correctly under session-based agreements is usually the hardest money problem for these owners. We calculate clinician pay every pay run based on the agreements you already have, then run payroll so everyone is paid on time.

Common Questions About This Area

Do you work with practices in Denver?

Yes. Our office is in Lenexa, Kansas, and our remote division serves independent practices nationwide, including throughout Denver and the surrounding metro. We work in QuickBooks Online and Gusto, so documents, approvals, and meetings all happen online.

I'm leaving a health system to open a direct primary care practice. When should I call you?

A few months before you open is ideal. We help set up the business structure, books, payroll, and software so everything is ready on day one. New practice setup runs $500 to $5,000 one-time depending on what you need.

Can you handle Denver sales tax on the products we sell?

Yes. Denver collects its own sales tax separately from the Colorado state return, and several nearby cities do as well. We file on supplements, skincare, and other retail products in every jurisdiction where you owe.

Do you set clinician pay rates for behavioral health groups?

No. We calculate pay based on the compensation agreements your practice already has and run the resulting payroll. We don't design compensation plans, because clinician pay is subject to medical compliance rules. What we do is make sure every clinician is paid correctly and on time.

What if our books are behind?

That's a common starting point. We bring the books current first, then move into monthly service. Monthly bookkeeping runs $500 to $1,500 per month, and catch-up work is 75% of the monthly fee for each month behind.
