Bookkeeping and Tax for Orlando Medical Practices

Bookkeeping, payroll, and tax for Orlando med spas, independent practices, and direct care clinics. Remote service from a firm doing tax work since 1995.

Bookkeeping and Tax for Orlando Practices

Orlando keeps adding practices. New med spas are opening in Dr. Phillips and Winter Park. Physicians are leaving hospital employment to start direct primary care clinics. Specialty groups are setting up near Lake Nona. Many of these owners are running a business for the first time. We handle the bookkeeping, payroll, and tax work so the financial side keeps up with the growth.

Florida has no state personal income tax, and that is a real advantage for practice owners. It also means almost all of the planning happens on the federal return. Every year, our enrolled agents review entity structure, owner pay, retirement contributions, and the timing of equipment purchases. That planning is the core of our tax services for physicians and other practice owners in Orlando.

Monthly Bookkeeping

We close your books every month and send a one-page summary. If your practice bills insurance, we check that what your billing system says you collected actually reached the bank. If your books are behind, we bring them current first.

Opening a Practice

Our new practice setup service is for physicians and owners opening in the Orlando area. We set up your business structure, books, payroll, and software before your first patient arrives, so you are not sorting it out during your first busy month.

Payroll

We run staff and provider payroll through Gusto. Florida has no state income tax withholding, but reemployment tax still applies and comes with its own quarterly reports. We handle those along with federal deposits and year-end forms.

Tax Returns and Planning

We prepare returns for the practice and for you as the owner, along with quarterly estimates and a yearly plan to lower what you owe. Enrolled agents handle any IRS notices. They are federally licensed to represent you before the IRS.

Orlando Practices We Work With

Orlando’s healthcare economy has two sides. Large systems like AdventHealth and Orlando Health employ thousands of clinicians, and Lake Nona has grown into a medical hub around the VA medical center and the UCF College of Medicine. At the same time, independent practices keep opening across the metro, and many of them are cash-pay.

Our firm was founded in 1995 in Lenexa, Kansas, and we work with Orlando clients remotely. Our Chief Information Officer, Marc Eastin, is a practicing neurosurgeon who also owns a business. He designed our systems around how little time a working physician has for paperwork.

Med Spas and Aesthetic Clinics

Orlando’s med spa market gets more crowded every year. Our med spa bookkeeping shows your profit on each treatment after product cost and commissions. We track prepaid packages and memberships so your revenue isn’t overstated, and we file sales tax on retail skincare.

Direct Care and Concierge Medicine

Many of these practices are run by physicians who left hospital employment to open membership practices. They bring membership revenue, retail supplements that may be taxable, and a first year of running a business. We set it up correctly and keep it current.

Independent Physician Practices

We work with primary care, dermatology, surgical specialties, and other physician-owned groups across the metro. We check insurance collections against bank deposits and calculate provider pay from your existing agreements. If you own your building, we keep separate books for it.

Behavioral Health and Therapy

Therapy and counseling groups are growing with demand across Central Florida. We calculate session-based clinician pay from your agreements every pay run. We also help with hiring, led by our President, whose background is in clinical psychology and recruiting.

Common Questions About This Area

Do you have an office in Orlando?

No. Our office is in Lenexa, Kansas, and our remote division works with practices across the country, including Central Florida. You send documents through a secure portal, and we meet by video when it fits your schedule.

Florida has no state income tax. Do I still need tax planning?

Yes. For most practice owners, the federal return is still the largest tax bill by a wide margin. Entity structure, a reasonable owner salary, retirement contributions, and the timing of equipment purchases all change what you owe. Florida also taxes C corporations at the state level, which is one more reason to choose your structure carefully.

I moved to Orlando from another state. Does that affect my taxes?

Usually, yes. In the year you move, you may need to file a part-year return in your old state for income earned before you left. We sort out which income belongs where. Some states look closely at high earners who leave, so we also help you keep clear records of when you moved.

What does bookkeeping cost for an Orlando practice?

Monthly bookkeeping runs $500 to $1,500 a month depending on transaction volume and complexity. If your books are behind when we start, catch-up is billed at 75% of the monthly fee for each month behind. We quote a flat rate after we look at your books.

Does my med spa need to collect sales tax?

Usually, on retail products like skincare and supplements. Orange County adds a local surtax on top of the Florida state rate. We track what is taxable, file on schedule, and keep sales tax from turning into an unexpected bill.
